Student Assistance and Support Form

This form was created for members of our program community (i.e., students, faculty/instructors, staff, and partners) to share any observations or concerns that may have an impact on our academic and/or interpersonal graduate enterprise. The MS in Applied Data Science Student Affairs Team, in partnership with the Physical Sciences Division Dean of Students, works directly with students to identify solutions, coordinate resources, and/or implement a centralized response rooted in care to assist & support students within our program community. 

Curricular Practical Training (CPT) Requests

This form is intended for any full-time student, currently with a student visa, interested in doing an internship through CPT. Submit a request to obtain a signed CPT recommendation, which is required to apply for CPT via the Office of International Affairs (OIA).

Course Substitution Requests

This form allows current students to submit a request to use an alternative course to meet a program elective requirement. 

Course Overload Requests

Students who wish to take more than 300 units in any given quarter must have permission from the MS in Applied Data Science Advising Team. MS in Applied Data Science students are limited to 400 units in a quarter, if approved for a course overload. Unit overloads are considered on a casebycase basis and may be denied.

Intent Not to Enroll

The MS in Applied Data Science Advising Team is requesting that any current student that is planning not to enroll in courses for any given quarter, please complete this form. If you are currently on active leave of absence, this form does not apply to you.

Request for Registration Changes 

This form is intended for current students to request registration course changes (add/drop/swap) beginning Week 2 of each quarter to the MS in Applied Data Science Advising Team.

Closed Course Request Form

Requests for consideration into closed courses are managed by the MS in Applied Data Science Advising Team. Do not contact individual instructors to request consent to be added. Closed course requests are considered on a first come, first serve – case-by-case basis and may be denied. Submission does not guarantee enrollment or the increase of closed course capacity.
